Attached below are the new volunteer forms. Everyone, including CCRC members, must fill out the CRWRC Group Volunteer Application Form' as well as the 'CCRC Waiver and Release of Claims Form'. Please print out the 'GROUP VOLUNTEER FORM' (forms will also be made available at church every Sunday) and fill it out. Once completed, please fax the form to CRWRC-DRS to the attention of Art Opperwall 616-726-1141 or scan and email to aopperwall@crwrc.org. This form must be faxed or emailed to CRWRC-DRS at least ONE WEEK prior to your group's arrival. Our volunteer coordinators Nhen Prum and John Mullen will follow up with you as soon as the application is received to confirm and coordinate your arrival/volunteer time. Volunteer Group Leaders please make the 'Medical Information' form available to your group members to fill out. CRWRC-DRS recommends that this form be filled out by all group members and then kept by the group leader for the duration of the trip. In case of medical emergency the forms could serve as important information for medical providers- should the need arise. These forms do NOT need to be sent to CRWRC-DRS or CCRC. They should be kept confidential by the group leader and be used only as needed for medical treatment. The CCRC requires that persons be age 16 years or older in order to volunteer at the church construction site due to safety concerns. But we would greatly appreciate your prayer support or help in auxiliary areas!
